Member Rights & Responsibilities 

Members have the RIGHT to:
  • Be treated with respect and dignity
  • Privacy and confidentiality
  • Access to current information on services covered or not covered by their plan of benefits
  • Instruction on how to file a complaint or an appeal
  • Timely and effective address of complaints, appeals and/or grievances
  • Access to information contained in medical records, unless clinically contraindicated
  • Confidentiality of medical records unless disclosure is necessary to interpret coverage under a specific plan
  • Be informed of appropriate or medically necessary treatment options for their condition
  • Obtain emergency services when necessary, and without delay
  • Make informed decisions about treatment
  • Refuse treatment, but accept the consequences of the decision
  • Receive a copy of MediCor’s rights and responsibility statement
Members have the RESPONSIBILITY to:
  • Read and understand their plan of benefits
  • Pay deductibles and copayments required by the plan
  • Present member I.D. card to health care professionals when seeking care
  • Respect treatment providers and staff
  • Avoid dishonest activity directed to the plan or any healthcare provider
  • Obtain authorization for services under your plan that require it
  • Follow the directions and advice of the healthcare professional
  • Accept the consequences of financial or clinical outcomes when recommended treatment by a professional is refused
